As nurses, we’re often trusted not only for our clinical expertise but for our ability to guide patients and families through emotional, personal decisions. One of the most important conversations we can support is around advance care planning—specifically, living wills and power of attorney for healthcare.


Here’s how to approach it with empathy, clarity, and professionalism:

1. Create a Safe and Supportive Space

  • Begin the conversation in a quiet, private setting.

  • Use open body language, a calm tone, and reassuring words.

  • Acknowledge that these decisions are difficult but deeply valuable.


2. Explain the Purpose Clearly

  • Living Will: A document that outlines what medical treatments a person would or would not want if they cannot speak for themselves.

  • Power of Attorney (POA) for Healthcare: Appoints someone to make medical decisions on their behalf if they’re unable to.


3. Normalize the Conversation

  • “This isn’t about giving up—it’s about planning ahead so your wishes are respected.”

  • “Many people feel a sense of peace after putting these choices in writing.”


4. Encourage Personal Reflection

  • “What matters most to you if you were seriously ill?”

  • “Are there treatments you would want—or want to avoid—if you couldn’t speak for yourself?”
